Community Paramedicine Program

In the District of Timiskaming Social Services Administration Board (DTSSAB) area, the Community Paramedicine program collaborates with local health system partners. This includes working with home and community care, primary care, emergency care, and other community and social services. The program is designed to be accessible, responsive, proactive, and safe.

HOME VISITS

During a home visit, a Community Paramedic typically performs a range of tasks aimed at providing comprehensive healthcare and support to the patient. These tasks can include:

  1. Assessment: Conducting a thorough assessment of the patient's overall health status, including vital signs, mobility, cognitive function, and medication management.

  2. Fall Risk Assessment: Evaluating the patient's risk of falls and implementing measures to reduce this risk, such as recommending home modifications or providing education on fall prevention strategies.

  3. Chronic Disease Management: Monitoring and managing chronic diseases such as diabetes, hypertension, COPD, or heart failure. This may involve checking blood glucose levels and/or providing education on disease management.

  4. Medication Management: Ensuring medication compliance, reviewing medication regimens, and addressing any concerns or questions the patient may have regarding their medications.

  5. Education: Providing educational resources and information to the patient and their family members on topics such as nutrition, exercise, medication management, and disease prevention.

Wellness Clinics conducted by Community Paramedics within the community are offered monthly at various locations across Timiskaming District. These clinics play a vital role in promoting health and wellness, focusing on preventive care, health promotion, and education to improve the overall well-being of community members. Services such as health screenings, wellness assessments, and chronic disease management are provided, helping individuals proactively manage their health and reduce the risk of developing health issues.
